Salam, I embraced Islam last year and shortly after I went into a psychosis. This resulted in me jumping from an 8-meter tall building and was confined to a hospital for 7-8 months. During this time I lost my iman. Now that Ramadan is here I feel guilty and long for Allah SWT to help me. I started talking to Him and just got a feeling of worry. Praying is a problem because of my injuries, but more importantly my reluctance to do it. I embraced Islam to quickly and learned it the wrong way.
If Allah SWT wills, he will help me, but I ask you, brothers and sisters, to tell me that taking it slow and figure things out is okay. I don't want to rush into it like I did last time and end up damaging my relationship with Allah SWT even more.
Will He forgive me for the sins I did the past year if I embrace Islam correctly?

I am so sorry to hear of the struggles you have gone through over the course of this year, mental illness is something very difficult to deal with and I wish you the best of luck in your recovery and growth in upcoming months. I understand your challenges in not being able to fast over the course of Ramadan, however Allah swt is first and foremost Most Gracious and Most Merciful. Just like a cancer patient is released from his obligation to fast during this holy month, fasting can have severe impacts on the ill and Allah swt as all-knowing will be understanding with you. Make sure that you are taking your medications and staying healthy and doing Salah when you can, going slow and taking your time with God and faith will be healthier for both you and your community. Hey sorry to hear about your difficulties. Perhaps you don't need to turn to Islam. Try to look into some of the errors in the quran before you commit again. Look at the criticisms with an open mind. Think for yourself, don't turn to an old book of stories.

We were in the same position. A few months after my conversion I was diagnosed with diabetes the day before I went in for surgery to remove a painful cyst. It took a month to recover. I dropped classes, missed work, missed deadlines for submitting papers to social services (for food stamps). I was alone, scared, and a little angry. I turned my back on Allah but I came back last week. Take some time and consider if you can live without Allah.
